59 PM EasternDescription The City of Norfolk is seeking a Service Coordinator/Case Manager III -Infant Development to join the Norfolk Community Services Board.
This position provides Service Coordination/case management services to children, birth to three years of age, with developmental delays, diagnosed conditions or atypical development & their families by assessing, planning, linking, monitoring, and advocating for them in response to their changing needs.
Completes clinical documentation in accordance with agency and regulatory guidelines.
The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities.
Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.
Today, the CSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly.

Departmental Hiring Salary Range:
$45,013 - $49,514 Essential Functions Essential functions include but are not limited to:
Accepts Early Intervention referrals from multiple sources.
Provides Early Intervention Service Coordination from initial referral through transition/discharge.
Develops an Individualized Family Service Plan (IFSP) based on assessment information conducted by a multidisciplinary team & in collaboration with the client's family and significant others.
Reviews and updates the IFSP at least every 6 months unless needed sooner.
Conducts a complete revision of the IFSP annually.
Complies with regulations for HIPAA, mandated reporting of abuse and/or neglect, Client Rights/Part C Procedural Safeguards and Client Confidentiality.
Complies with Part C, Medicaid, DBHDS, Agency requirements and all applicable regulations.
Maintains chart documentation that is in compliance with Part C, Medicaid and agency requirements.
Ensures that documentation substantiates billing by completing documentation and submitting filing within 3 days of providing the service.
Completes a daily record of direct and client related service.
Contact with the family and/or service providers is completed on a monthly basis in accordance with regulatory guidelines.
Face to face visit with the child to assess ongoing eligibility for service, progress toward meeting IFSP outcomes, monitoring of IFSP services, family/consumer satisfaction as well as an assessment of the child's physical and mental status is completed as often as clinically necessary and in compliance with regulatory guidelines.
Makes contact with family to promote implementation of the IFSP and to ensure community stability.
Identifies and links the children and families to services, resources & supports specified in the child's IFSP.
Assists families in procuring and maintaining resources and entitlements.
Assists the family directly, which may include transportation, for the purpose of obtaining needed resources or services, while empowering the family to be as independent as possible.
Monitors progress, service delivery, physical and mental stability and client satisfaction by providing monthly service to all individuals on assigned caseload.
Provides increased frequency of service whenever clinically indicated.
Coordinates services and service planning with other providers within the program, NCSB, other agencies and private providers etc.
and obtains clinical documentation from other providers to ensure continuity of care.
Provides practical counseling and education to guide the family/caregiver in problem solving strategies and to develop a supportive relationship.
Performs crisis intervention to resolve critical situations.
Updates and/or maintains clinical data, in accordance with agency requirements, for consumers on assigned caseload.
Attends agency staff meetings, supervisory meetings, child staffing meetings and training as required.
Maintains EI Service Coordinator certification.
Performs other related duties as assigned.
Education/Experience Bachelor's degree in a human services field.

This will be a pre-tax payroll deduction.
If you are hired January 1, 2022, or after to a position with Virginia Retirement System (VRS) benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit.

The Tuition Assistance Program is established to encourage employees toward continued self-development and education.
